Juvenile idiopathic arthritis

Posted in Medical

Juvenile idiopathic arthritis (juvenile chronic arthritis) is a term used to describe a clinically heterogeneous group of idiopathic arthritides occurring in children under 16 years of age; subtypes include systemic arthritis, oligoarthritis (particularly of leg joints), and polyarthritis, either positive or negative for rheumatoid factor.

Treatment involves many of the same drugs used for rheumatoid arthritis in adults (see below), although there may be limited evidence for their use in children. Appropriate management will depend on the subtype of disease, but there is no definitive consensus on the drugs of choice.

Fever and hyperthermia

Posted in Health

Body temperature is normally regulated by the hypothalamus. Fever (pyrexia) is a controlled increase in body temperature due to an elevated hypothalamic set-point temperature. Common reasons for this include infections, inflammatory disorders, neoplastic disease, and some drug treatment. Hyperthermia (hyperpyrexia) implies a disturbance of thermoregulatory control.

It is caused by external factors such as injury to the hypothalamus, heat stroke following defective heat loss (as occurs in dehydration or excessive heat production following strenuous activities), excessive dosage of some drugs, or a reaction to certain drugs such as anaesthetics (malignant hyperthermia) or antipsychotics (neuroleptic malignant syndrome). Underlying thermo regulatory defects may be a particular problem in sedentary elderly subjects.

Drug Courts

Established as a result of court and prison overcrowding, special drug courts have proven popular. The effects of aggressively expanding punitive drug enforcement starting in the 1980s have been cited as one of the main rationales for the vigorous emergence of drug courts. “The enforcement-centred ‘War on Drugs’ has sent large populations of drug offenders into the criminal justice system, and has led to an unprecedented increase in the number of offenders receiving long imprisonment sentences”.

In 1989, a special drug court was established by judicial order in Miami, Florida. This high-volume court expanded on traditional drug defendant diversion programs by offering a year or more of court-run treatment; defendants who complete this option have their criminal cases dismissed. Between 1991 and 1993 Miami influenced officials in more than twenty other jurisdictions to establish drug courts.
